Instructions

To claim CME credit, please follow these steps:

1. Answer all evaluation questions. Enter NA for questions not applicable or for which you do not have feedback.
2. When you are finished with the evaluation, click the "Save" button in the lower right corner to record your progress (the window will close).
   YOU ARE NOT FINISHED! You must proceed to the next step to claim CME!
3. You will be brought back to the "My Portfolio" page.
4. In the "My Activities" section, click the "Claim Credit" link next to 2013 Annual Assembly. 
5. Enter the amount of CME credits you wish to claim, which should be commensurate with the extent of participation in the attended activity and click "Claim".
